From what I read, he took her to the seaside after escaping from the hospital with her and she died. Then they show later on where he is working as a resident in a hospital with the older guy/doctor from before. We see that he's married and apparently his wife has the same interests as Kurumi. It appears that her "ghost/spirit" is with him also.

I quite like this discussion here; reading it kind of gave me some relief. However, there's an additional opinion I want to share.

I see that this manga is not very popular, but in most recommendations, people highlight the ending with comments like, "Look at that ending!" For me, it wasn’t so straightforward. After the first few volumes, I even forgot what genre I had picked up, though it turned out to be exactly what I was looking for. Perhaps I fell victim to the "professional assassin" as well—what a trap for the reader! I began to think that I read Japanese version of "American pie" or something similar.

On a second thoughts, after the initial emotional roller-coaster (spoilers ahead), I found the OBs to be incredibly cynical and harsh toward the club members, especially the presidents. The ex-president was brought down by a precisely targeted strike into his fetish, with the OBs ma asking fun of this.

For the second president, Aiba, they sent a carefully chosen "assassin" who matched his personal preferences in both appearance and behavior. She began her cold, calculated work of moral suppression and submission, but things took an unexpected turn. Sometimes Aiba decided not to succumb but to grow stronger and better, showing her that life is full of choices and possibilities—not just a set path of following orders and expectations. I think this is why Kurumi started developing feelings for him and began enjoying her life, even surprising the OBs, who noticed the change during the festival.

The final volume offers many clues about Sahana’s backstory, including the mystery of her age. One significant hint involves the orphanage scene, where the author shows a young woman, likely in her twenties, who seems to sense Kurumi's presence even from a distance. This suggests someone who was very close to her—probably someone who spent their childhood in the orphanage with her, forming a deep bond. If that’s the case, our cold-blooded "professional assassin" is likely around her twenties as well.

Another detail stands out: when Kurumi is shown bare, she is without pubic hair. This scene is both an emotional and a cynical twist. Despite being a developed girl, at least 16 years old (as implied by her periods), she was deliberately prepared and shaved, presumably as part of the OBs’ plan to "bag" Aiba—similar to what was orchestrated with Katsu and Kyouko during the UFO hunt in the mountains. However, our innocent boy as usual just got overexcited by being with Kurumi and that not happened. Worth to mention - he was not able to conduct any sexual move without order or approval from Kurumi, and she didn't told him to do anything. Taking in account all mentioned, it makes look like even timing of passing away Kurumi was plotted but OB's

In the end, Aiba passes the test, becoming a better version of himself while addressing the sexual deviations rooted in his past, in Kurumi. His adult couple is just another reassemble of his taste in girls showed in first volume.

Ultimately, the story unfolds as a twisted, psychologically charged horror. The only lingering question for me is understanding Kurumi’s true mindset. I doubt a second read-through will help me unravel her completely.
